WorkCover
All of our physiotherapists are WorkCover accredited and can help you recover from your workplace injury. Your Worksafe Insurer will cover the full amount of your physiotherapy treatments here at Revive. Our accounts department will liaise directly with all the major insurers so there will be no paperwork or extra things for you to chase up.
It is important that you bring your GP referral, claim number and date of injury, as well as any insurance company information, case manager details and/or the contact details of your workplace workcover representative with you to your first appointment so that we are able to invoice the insurer directly process the claim on your behalf.

TAC
If you have suffered an injury whilst on the roads in Victoria, you may be entitled to assistance with the cost of your physiotherapy via the TAC. At Revive, we will invoice the TAC directly, with no out of pocket costs to you, the client. Please remember to bring your claim number and date of accident to your first appointment so that we are able to process the claim on your behalf. If your accident has occurred after 14th February 2018, you are no longer required to meet a medical excess prior to TAC funding your treatment.

Patient-centred Treatment
DVA
At Revive, we are able to provide treatments for Department of Veteran Affairs (DVA) Gold Card holders, and for approved areas for White Card holders. We bill DVA directly for all treatments and there is no out of pocket cost to you, the client. Please ensure that you bring a current DVA referral from your GP to your first appointment.

Medicare EPC
Enhanced Primary Care (EPC) is a Medicare initiative which allows GPs to refer patients to Allied Health services for a rebated cost if they have a chronic medical condition. Medicare card holders are entitled to 5 of these sessions every calendar year.
These sessions can be a mixture of Allied Health services including physiotherapy, podiatry, exercise physiology or dietician for example. Once you have used your 5 Allied Health sessions in a 12 month period, any subsequent consults will be fully paid by yourself, with no rebate claimable from Medicare.
In order to maintain a high level of service and uphold our 30-minute duration appointment times, we require you to pay a small gap. Please note, the gap must be paid on the day, and is not coverable by private health insurance.
Consultations must be paid in full and then we can lodge your claim electronically so you are only responsible for paying the gap – no paperwork to chase up! The first time you come to visit, please bring your Care Plan which is signed and dated by your doctor.
