At Revive we provide health services for people who are self managed and plan managed through the National Disability Insurance Agency to become empowered to achieve their full potential through evidence based physiotherapy.

We work with NDIS participants to help them achieve their full potential and maximise their quality of life. At Revive, we treat a broad range of conditions ranging from paediatrics, pain, neurological and musculoskeletal conditions. Whether your issue is pain, decreased strength, mobility, brain injury or another musculoskeletal disorder, we can work with you to achieve your goals. We are passionate about helping people of all ages to make daily tasks easier and to help you get more out of life.

Who can we help?

We can currently help those who are self managed and plan managed through the NDIS. How the billing process will work will depend on how your funds are managed.

If you are plan managed we will require the details of your plan manager and support coordinator, as well as your NDIS client number and any billing information. We will be able to send invoices directly to your plan manager, so all you have to worry about is attending your appointment.

If you are self managed the process is slightly different. In this instance, you will be able to pay for your appointments using your funding and we will then provide invoices so you can keep track of everything you need for the NDIA.

Our physiotherapy appointment pricing falls in line with the NDIS dictated rates. Your initial consultation will generally be 45 minutes to 1 hour, after which time you have the flexibility to choose if you prefer 30 minute or 45 minute follow up consultations.

Our Physios can help treat and manage:
  • Pain
  • Neurological conditions, such as Parkinson’s Disease, Cerebral Palsy or Multiple Sclerosis
  • Developmental delays in children

  • Chronic diseases, such as diabetes, osteoarthritis, osteoporosis and obesity
  • Stroke
  • Rehabilitation after surgery
How we help

Your physiotherapy treatment will be tailored to your individual needs so that we can work together to help you achieve your goals.

Treatment may include:

  • Manual therapy such as soft tissue massage and joint mobilisation to reduce pain and stiffness
  • Exercise programs to improve mobility and strengthen muscles
  • Muscle re-education to improve control and movement efficiency
  • Clinical Pilates
  • Dry Needling
  • Hydrotherapy
  • Advice and assistance with the use of mobility aids, such as walking sticks, walking frames and braces

WorkCover

All of our physiotherapists are WorkCover accredited and can help you recover from your workplace injury. Your Worksafe Insurer will cover the full amount of your physiotherapy treatments here at Revive. Our accounts department will liaise directly with all the major insurers so there will be no paperwork or extra things for you to chase up.

It is important that you bring your GP referral, claim number and date of injury, as well as any insurance company information, case manager details and/or the contact details of your workplace workcover representative with you to your first appointment so that we are able to invoice the insurer directly process the claim on your behalf.

Looking for TAC Services in Altona North?

TAC

If you have suffered an injury whilst on the roads in Victoria, you may be entitled to assistance with the cost of your physiotherapy via the TAC. At Revive, we will invoice the TAC directly, with no out of pocket costs to you, the client. Please remember to bring your claim number and date of accident to your first appointment so that we are able to process the claim on your behalf. If your accident has occurred after 14th February 2018, you are no longer required to meet a medical excess prior to TAC funding your treatment.

DVA

At Revive, we are able to provide treatments for Department of Veteran Affairs (DVA) Gold Card holders, and for approved areas for White Card holders. We bill DVA directly for all treatments and there is no out of pocket cost to you, the client. Please ensure that you bring a current DVA referral from your GP to your first appointment.

Looking for EPC Services in Altona North?

Medicare EPC

Enhanced Primary Care (EPC) is a Medicare initiative which allows GPs to refer patients to Allied Health services for a rebated cost if they have a chronic medical condition. Medicare card holders are entitled to 5 of these sessions every calendar year.

These sessions can be a mixture of Allied Health services including physiotherapy, podiatry, exercise physiology or dietician for example. Once you have used your 5 Allied Health sessions in a 12 month period, any subsequent consults will be fully paid by yourself, with no rebate claimable from Medicare.

In order to maintain a high level of service and uphold our 30-minute duration appointment times, we require you to pay a small gap. Please note, the gap must be paid on the day, and is not coverable by private health insurance.

Consultations must be paid in full and then we can lodge your claim electronically so you are only responsible for paying the gap – no paperwork to chase up! The first time you come to visit, please bring your Care Plan which is signed and dated by your doctor.
